What is a Patient Centered Medical Home (PCMH)?

This designation means that we are constantly striving to improve our procedures and processes in order to provide the best and highest quality care to your child. 

We strive to provide evidence-based care, patient/family education and support. We provide equal quality of care and services for all insurance types that our practice accepts. In order to be a “true” medical home, we need your help.

Please think of us as the first place to come for advice and care regarding all of your child’s health care needs. This includes physical health, development, emotional health, as well as a place to help you find the appropriate specialist or access community resources.

We also want to help coordinate your care. Let us know if you have been treated by a specialist, an emergency department or any outside healthcare professional, including therapists and counselors. We routinely request and process information from others who are involved in your care. Informing us of medications, test results and procedures is key to having a complete understanding of your child’s health. Every time you are seen for care outside of our office, please request that a copy of the visit or test result is sent/faxed to us so that we can best serve you. In addition, please feel free to call our office about care you have received. You should be prepared to update us at all office visits, whether they are for an acute problem or a routine well-visit. You may call our front office staff members at
206-783-9300 and choose option 6 to coordinate transfer of medical records.

It is also very helpful to us if you let us know who you consider to be your primary care provider (PCP) or doctor. This physician is listed in your chart and all information that is transferred to our clinic is sent to that provider. Designating a PCP enables our providers to have a full understanding of your health. Good care requires a partnership between your family and our office. We appreciate your help and look forward to working with you.
