Do you or your child have symptoms? 

Cough, runny nose, fever, sore throat, congestion?

Have you had a known exposure within the past 10 days?

Please put on a mask

No symptoms?

Masking is not required

Do you want your medical assistant,

nurse or doctor to wear a mask?

No problem!

Please ask and we are happy to wear one.

This is subject to change depending on respiratory illness

in the coming months.
